St. Luke is located in the City of Oswego, overlooking the scenic Oswego River. We are dedicated to helping the people we serve live to their maximum potential and promote dignity of life throughout all of its stages.
St. Luke is a trusted and experienced provider of residential skilled nursing and short-stay physical/occupational rehabilitation services serving the Oswego County area since 1975. Turn to us if you need Palliative Care, Wound Care or IV Antibiotic Therapy services, provided by caring, experienced professionals.
Our services extend into our community. Our adult day programs both St. Luke's "medical model" Adult Day Health Care, and The Cornerstone Club, a social adult day program offer supportive services and activities to help older adults remain in their home settings.
For help in the home our Home Health Services provides personalized services to meet your individualized needs in your home setting, all designed to help you maintain or achieve maximum independence. Our New York State Department of Health licensed home care agency serving Oswego, Jefferson and Onondaga counties.
St. Luke Health Services is a part of The St. Luke Family of Caring, an affiliation of community-based, not-for-profit healthcare providers serving the greater-Oswego County area. Affiliates also include Bishop’s Commons Enriched Housing Residence and St. Francis Commons Assisted Living Residence. Together we provide care and services to over 300 people daily.
