Meeting You Where You Are is the theme for this year’s celebration. We, the Palliative Care Team at St. Luke are always ready to meet our patients and their loved ones where they are.

From the comfort of lending a hand at bedside, from the solace of a reassuring voice to enhancing access to distant family, friends or resources, we always aim to do our best to cater to our patients' needs.

Palliative care is medicine at its BEST. It is holistic and comprehensive medical care for people afflicted with a serious illness. It is based on a patient’s needs and its aim is to offer and provide relief from the burdening symptoms that go along with illness. The goal is to improve and enhance the quality of life for both the patient and the family. It is appropriate at any age and at any stage of a serious illness.

At St. Luke, we pride ourselves on our Palliative Care Program and our Palliative Care suite. Our team consists of various disciplines; from medical, nursing, activities, dietary, and maintenance staff. Through our various backgrounds and the nature of our work, we are all united in our goal to provide what our residents need.

Please continue to join us in our efforts to promote and foster Palliative Care in our facilities and community. With our Hospice and Palliative Care Certified Medical Director and our Certified Palliative Care Nurse, we strive to provide best practices that are consistent with national guidelines.
