Adult Day Health Care

As the most experienced provider in the area, we launched the region’s first “medical model” Adult Day Health Care program in 1993.

Our program offers daily health monitoring and activities designed for disabled adults 18 years or older to promote independence and delay institutionalization.

Located at St. Luke Health Services in Oswego, we provide a supportive, easily accessible, and friendly atmosphere for ongoing rehabilitation, medical services, and quality health care.

Our services are tailored to each registrant's needs and include:

  • Case management
  • Health assessment
  • Physical rehabilitation therapy
  • Speech therapy
  • Therapeutic diets

Our Adult Day Health Care is a Medicaid eligible program serving all of Oswego County.  We’re open Monday through Friday, and registrants may attend one to five days per week, as preferred.
