Chapter 114 subdivision 12 of section 2803 of the New York State Public Health Law requires that each residential health care facility in New York prepares and makes available a copy of their Pandemic Emergency Plan (PEP) to the public.

Adult Day Health Care at St. Luke is a "medical model" adult day care program. Registrants receive daily health monitoring and activities program designed to promote independence and wellbeing for adults.
CAPABLE (Community Aging in Place—Advancing Better Living for Elders) is a client-directed, home-based intervention to increase mobility, function, and capacity. A unique eveidence-based program developed by the Johns Hopkins School of Nursing, CAPABLE helps older adults maximize their strengths and resources to safely live home.

St. Luke Home Health Services is licensed home care agency providing personalized services to meet your needs in your home setting. Whether you suffer from a chronic illness, or are recovering from a recent surgery or other medical event, we provide the flexibility to assist you as changes occur in your health.
Our interdisciplinary teams at St. Luke provides IV antibiotic therapy for medically complex residents who require antibiotics to treat infection. We offer care for residents who may be seriously ill and experiencing multiple medical problems, including many currently on IV medications in an acute care setting.
Palliative care is available through our Comfort Care Program. Offered at St. Luke, Comfort Care provides compassionate care and support for residents and families of those facing a serious illness, or who are in the final stages of life.
Short-Stay is a comprehensive sub-acute rehabilitation program offering physical, occupational, and speech therapy, provided at St. Luke. Our goal is to return individuals safely to the community following an illness, accident or elective surgery. Our team of professionals collaborates with patients and their families to help achieve the highest level of function for each individual in our care.
St. Luke has been serving the healthcare needs of the greater Oswego County community for over four decades. We pride ourselves on providing the highest quality twenty-four hour skilled care in a friendly and homelike environment with comfortable and familiar surroundings.
Our Wound Care and Healing Program is available to both Short-Stay Rehabilitation patients and long-term skilled nursing care residents. Our program specializes in the treatment of complex and hard to heal wounds that can result from a variety of acute or chronic conditions including venous ulcers, arterial and diabetic wounds, pressure ulcers, surgical wounds or traumatic injuries.
We routinely provide free health screenings in various locations throughout Oswego County. We also host or sponsor services and educational programs that help make our community a healthier place for everyone.
